What is needed for us to assist you to schedule your state test:

• Correctly completed State Test application form including cash only to test in our site or completed credit card information on page 2 to test at another site
• Copy of program certificate
• For retake: copy of test results letter from D and S (may be obtained from online)showing which portion of the test has not been passed and the appropriate payment method

PLEASE CALL FIRST TO CONFIRM THAT SOMEONE WILL BE ABLE TO ASSIST YOU ON THE DAY YOU ARE COMING IN TO REGISTER AS WE ARE FREQUENTLY OUT IN THE FIELD.

To view your state test results online:

go to: Ohio STNA

Click: "Online Test Results" at lower left side of page

Enter: Social Security Number, the date you took your test (as 00/00/0000 format) and your date of birth (as 00/00/0000 format) 

Click: Submit Score Report Request

It will ask you if you want to only view the web page content that was delivered securley.  Click "Yes" and you will see your test results IF they are available.
