Graduates of care4you2
Congratulations!
Signing up for State Testing
Once you have successfully completed your classes and have your certificate in hand, we would be happy to assist you in signing up for your state test examination. Our Toledo location at 901 Washington Street is our state testing site. You may choose to test at our site; to do so follow the instructions on the State Testing Dates page of this website.
To sign up to test anywhere other than the care4you2 test site, please go to hdmaster.com and choose an available test location and date. We would be happy to fax your paperwork in for you, however, you will need to use a debit or credit card for this registration and we cannot guarantee receipt of your materials by D and S nor can we guarantee availability of any test dates/times as we are not the ones scheduling for other locations.
Practice BEFORE the State Test
ALL care4you2 students are able to practice their skills in the Toledo site in order to be familiar with the site prior to your state test date.
Dates and times for skills practice:

May 6, 20

Please call us at (419) 345-4269 to discuss opportunities for you to practice.
All sites have practice written tests available for our students. Talk to your instructor about practice opportunities.
Upon successful completion of classes, care4you2 graduates are given a special web address so that they are able to re-review skills videos online in preparation for their state test examination. Special thanks to Dawn Ahrens, PC, and her Sylvania classroom students who made this possible!
Employment Opportunities
We ask that our students attempt to find employment on their own. We strongly recommend that you start by applying for employment where you completed your clinicals. However, if assistance is needed we are here to help!
We have created an employer listings web page for our graduates. Once our graduates have their STNA licenses and need our assistance for potential employment, we will provide them with a link to this site. This web page will constantly be updated with information of employers who have given us their permission to link their web sites to ours. With permission, employers who have either hired our graduates or employers willing to accept applications from our graduates will be linked to our website.
Ohio Nurse Aide Registry
Once you pass your Ohio state test, you should be able to locate your information at the website below.
https://odhgateway.odh.ohio.gov/nar/nar_registry_search.aspx
Please note the expiration of your state license so that you ensure you complete all the requirements PRIOR to expiration. You also want to make sure your employer reports your hours worked under a licensed nurse or physician to the state on your behalf. Remember: you need to work one 8-hour shift in the 24 month period AND complete 24 hours of continuing education (12 units per year) in order to maintain your state license for the state of Ohio. Please talk to your nurse instructor about any questions you may have about maintaining it once you obtain your Ohio license.
